For students who have already played a sport this school year, they may log into their account and follow these directions:

  1. Go to Section 8, answer the six (6) questions. If answer is “no” to all, sign and submit and they’re ready for the season. Both the student and parent need to log in to their own respective accounts and complete Section 8 to complete the forms on their own.
  2. If students check “yes” to any of the boxes, students will need Section 9 clearance from their doctor or from the school doctor on the day of the team physical.

Finally, be sure to complete the four (4) school forms: rules page, overnight stay page, reasonable suspicion page and hazing page to complete the information.

When the parent and student forms are done, students will be ready for physicals. The school district will offer physicals at each of the schools this year for the winter season.
