Required Immunizations
Immunization Requirements
Keep your child healthy and safe by staying up to date on their vaccinations. Pennsylvania law requires all students (K-12) be vaccinated while attending school. Students that DO NOT have the required vaccines may be EXCLUDED from school until they have received the appropriate vaccines or an exemption form and it has been submitted to the school nurse.
For attendance in all grades (K-12):
- Diphtheria, Tetanus and acellular Pertussis (DTaP) — 4 doses
- Polio (IPV) — 4 doses
- Measles, Mumps and Rubella (MMR) — 2 doses
- Hepatitis B (Hep B) — 3 doses
- Varicella (Chickenpox) — 2 doses
For attendance in 7th grade:
- Tetanus, Diphtheria and acellular Pertussis (Tdap) — 1 dose
- Meningococcal Conjugate (MCV) — 1 dose
For attendance in 12th grade:
- Meningococcal Conjugate (MCV) — Second dose
- DTaP — Fifth dose is not necessary if DTaP #4 was administered after the 4th birthday
- Polio — Fourth dose is not necessary if Polio #3 was administered on or after the 4th birthday and at least 6 months after the previous dose
- MCV — Second dose is not needed if MCV #1 was administered at 16 years of age or older
