Sports Requirements

 

Any student participating in school sports must have an initial athletic examination done by the school physician or the student's private physician per academic year.  Participation in subsequent sports requires recertification either by Health Services Staff or student's private physician.   

Athletic examinations done by a private physician cannot be completed until 6 weeks prior to the onset of practice for the particular sport.  The only exception is fall sports where private physicals done after June 1 will be accepted.

For middle schools, these completed forms must be submitted to the school nurse 2 weeks prior to the scheduled athletic examination if done by the school physician.  If the physical is done by a private physician, the signed forms should be returned to the school nurse as soon as the examination is completed.

For the high school,  these completed forms should be brought to the nurse's office at the time of the physical examination.  Papers will not be accepted prior to the examination.  If the physical is done by a private physician, the signed forms should be returned to the nurse's office during Activities Period on Tuesday or Thursday.  Recertification papers may be returned to the nurse's office during Activities Period.  Papers will not be accepted in the nurse's office at any other time.
