School Immunization Requirements

 

School immunization regulations, 28 PA Code, Chapter 23, Subchapter C, require all public, private, parochial, intermediate units and home schooled students show proof of immunization before attending school in Pennsylvania.

K or First Grade

Seventh Grade

All Grades 


Kindergarten or 1st Grade
:

The following minimum immunizations are required for all students entering school for the first time in K or first grade:

à        Diphtheria and Tetanus – Four or more properly spaced doses of DTP, DTaP, Td or DT, with one dose administered on or after the fourth birthday.

à        Polio – Three or more properly spaced doses of IPV or OPV polio vaccine.

à        Measles (Rubeola) – Two properly spaces doses of live measles containing vaccine (preferably MMR) with the first dose administered at 12 months of age or older, or measles immunity proved by serological evidence.

à        Mumps – One dose of live mumps containing vaccine (preferably MMR) administered at 12 months or age or older, or a physician diagnosis of mumps disease indicated by a written record signed by a physician or his/her designee.

à        Hepatitis B – Three properly spaced doses of Hepatitis B vaccine or a history of Hepatitis B immunity proved by laboratory testing.

à        Varicella – One dose of live varicella vaccine administered at 12 months of age or older, or chickenpox immunity proved by laboratory testing, or a written statement of history of chickenpox disease from a parent/guardian or physician.

Seventh Grade:

The following minimum immunizations are required for all students entering seventh grade. (or in an un-graded class for students in the school year that the student is 12 years of age).

à        Diphtheria and Tetanus – Three or more properly spaced doses of DTP, DTaP, Td or DT, or any combination of the three.

à        Polio – Three or more properly spaced doses of polio vaccine (IPV or OPV).

à        Measles (Rubeola) – Two properly spaced doses of live attenuated measles containing vaccine (preferably MMR) with the first dose administered at 12 months of age or older, or measles immunity proved by serological evidence determined by the hemagglutination inhibition (HI) test or any comparable test.

à        German Measles (Rubella) – One dose of live rubella containing vaccine (preferably MMR) administered at 12 months of age or older, or rubella immunity proved by serological evidence.

à        Mumps – One dose of live mumps containing vaccine (preferably MMR) administered at 12 months of age or older, or a physician diagnosis of mumps disease indicated by a written record signed by a physician or his/her designee.

à        Hepatitis B – Three properly spaced doses of Hepatitis B vaccine or a history of Hepatitis B immunity proved by laboratory testing.

à        Varicella – Age appropriate dose(s) of live varicella vaccine administered at 12 months of age or older, or chickenpox immunity proved by laboratory testing, or a written statement of history of chickenpox disease from a parent, guardian or physician.  (Two properly spaced doses of vaccine are required for children 13 years of age or older).

All Grades:

The following minimum immunizations are required at all grade levels:

à        Diphtheria and Tetanus – Three or more properly spaced doses of DTP, DTaP, Td or DT

à        Polio – Three or more properly spaced doses of IPV or OPV polio vaccine (Students 18 years of age and older are exempted.)

à        Measles (Rubeola) Two properly spaced doses of live measles containing vaccine (preferably MMR) with the first dose administered at 12 months of age or older, or measles immunity proved by serological evidence.

à        German Measles (Rubella) – One dose of live rubella containing vaccine (preferably MMR) administered at 12 months of age or older, or rubella immunity proved by serological evidence.

à        Mumps – One dose of live mumps containing vaccine (preferably MMR) administered at 12 months of age or older, or a physician diagnosis of mumps disease indicated by a written record signed by a physician or his/her designee.
